The European Centre for Disease Prevention and Control on Sunday released a new set of surveillance standards for respiratory viruses across the EU and European Economic Area, aiming to close gaps that have allowed outbreaks to gain traction before health systems could respond.
The guidance, described as the first integrated framework of its kind for the region, lays out practical objectives for tracking influenza, COVID-19, RSV, and other acute respiratory pathogens. It specifies the epidemiological and laboratory systems countries need to have in place, and explains how comparable data across borders can feed into national and EU-level prevention and control measures.
Rather than a single static document, ECDC built the guidance as a modular toolkit that will expand over the next year. A module on “right-sizing” — determining how many specimens to sequence and how often — is still in development, and future editions may cover wastewater surveillance and the use of electronic health records for early detection.
At its core, the guidance asks countries to maintain several layers of surveillance simultaneously. Primary care networks that track influenza-like and acute respiratory illness remain the front line for picking up early seasonal signals. Hospital-based systems monitoring severe acute respiratory infections provide a window into disease severity and healthcare strain. Laboratory databases, event-based reporting through the EU’s Early Warning and Response System, and mortality monitoring round out the picture.
The guidance also highlights newer approaches gaining traction across Europe. Wastewater monitoring, which expanded rapidly during the COVID-19 pandemic, offers an independent signal of community transmission that does not depend on people seeking medical care. Participatory surveillance — where volunteers report symptoms through digital platforms — captures infections among people who never visit a doctor, filling a blind spot in traditional systems.
The framework is designed to be flexible enough for countries with vastly different health infrastructure and surveillance histories. ECDC said the goal is not to impose a one-size-fits-all system but to give countries a common reference point they can adapt to their own context while keeping data comparable across the region.
Whether the guidance translates into better early detection will depend on whether countries invest in the laboratory capacity, data-sharing pipelines, and workforce needed to run these systems at the level the document envisions.
